Session 2

•    For each failure mechanism, estimate probability of occurrence and give scoring;
•    For each failure mechanism, define chain of effects:
o    Immediate effect on subcomponent
o    Subsequent effect on whole system
o    Final effect on end user;
•    Review severity score and relate to the chain of effects above;
•    Review detection mechanisms identified in FFMEA, where they act within the chain of effects, and give detection scoring.
